I have a 12 bolt that was pulled out of a 1969 Made in Canada Buick Skylark. The pinion yoke has a couple of chips broken out of it and I would like to get a new one. Which one is the correct one to buy?

Yep you need a picture. There are no aftermarket Canadian yokes which will have the 1 1/8" cup size with u-bolts...NOT 2 straps and 4 bolts normal 1 1/16" cup size (small block chevy 1310 big block chev 1330)

Looks about the same as that first pic and the bottom yoke in that third pic. I looked at it closer and its not really to bad. If not available might just go ahead and use it Pics:

checked the stash.i have 1 extra that i'd sell.after looking at yours,i'd just clean it up,by removing the nicks on the cup surfaces & reuse it.just don't overtighten the straps (very common).
