How the heck are you guy's emailing the form?? All I can do is download an XL file, hand write my info, then I would have to scan it in, send to email,etc.

Tom, it is a microsoft word doc.... if you don't have MS office, try to download Open Office ... huge file if you are on dial up

or call my cell and I will fill it out and e-mail it for you
